Instructions

When your PO has been picked, packed, and is ready to ship, complete and submit the form below. Once submitted, a BOL will be available for download with the assigned carrier. Please do not create BOLs more than 24 hours prior to shipping.

**NOTE:** A pickup is not automatically scheduled when the form is submitted. Please contact the assigned carrier to arrange a pickup from your location.
